1Connect Controllers
You must connect multiple controllers to be able to play co-op. If you're playing on PC and prefer to use a controller and one keyboard/mouse, toggle off the "Assign Controller to Player 1" option in the co-op settings.

2Select Characters
Once you're on the character selection screen, Player 1 will be the first one to choose. After a character is selected, it becomes unavailable to other players.

3Select Stage and Start
Proceed to your selected stage and begin playing with your friends! Take note that players will take turns leveling up, and if one of them dies, their level will transfer to the next player.

Can Co-op Be Played Online?
Poncle hasn't added an official way to play co-op online at the moment. However, they suggest that players can try using Steam's Remote Play Together feature.
Is Steam's Remote Play Together the only way to play Multiplayer?
You can play with your friend offline since Vampire Survivors is a couch co-op game.
Can I Play the DLCs in Co-op?
As long as players own the DLC, they will be able to play its content in this free multiplayer vampire game.
What's the Difference Between Solo and Co-op?
There are a few differences in solo and co-op gameplay to make it more challenging. For example, when a player dies in the local co-op, a coffin will appear, which counts down to their revival.
More Players, Lesser Weapon Slots
Another element added in co-op is that when more players join your party, each player will have fewer weapon slots available.

Friendship Amulet is a new item exclusive to co-op. The person who uses this item will grant a level up to all players on a random weapon. If all players have maxed out their weapons, the effect will be applied to a passive.
